—Team Name
—Team Captain and email address
—Team Roster (names, email addresses, and T-shirt sizes)
Registration team fee is $200. Team fee will help support CurePSP research initiatives.
To submit your registration fee, you can either:
- Register online with a credit card number.
- Send a check to CurePSP, 30 E. Padonia Rd, Suite 201, Timonium, MD 21093
- Call the CurePSP gift processor Kelly Saunders at 443-578-5670 (saunders@curepsp.org) with a credit card number.
